Healthcare is experiencing dramatic changes. The goal of this session is to demonstrate the dynamic nature of healthcare delivery and how the focus has shifted from care providers to patients. New models of healthcare delivery, such as ACOs and APMs, have emerged to fulfill the goals of the Quintuple Aim, which includes maintaining high-quality, low-cost, equitable care that engages and recognizes individual patients, improves the health of communities, and preserves the well-being of providers. As skilled nursing providers, we are seen as a powerful adjunct for delivering care to your residents and can positively impact the health of our surrounding communities, through collaboration with quality downstream partners. The FFS model is outdated, and new models of care delivery are required to fulfill the vision of the Quintuple Aim. CMS is committed to the expansion of ACOs as they have proven to be effective at driving collaboration across the network and have shown to improve patient outcomes, engagement, and value. Your facility’s capacity to create a roadmap that is ready for the future can be enhanced by comprehending these challenges.
